Before the Commonwealth Mobile Oral Health Services (CMOHS) team visits your location, we will need to distribute and collect dental consent forms. The dental consent form includes a medical and dental health history to be completed by the parent/guardian/patient (as appropriate). The CMOHS dental provider will review the medical history, take radiographs (as needed), examine the teeth and write a treatment plan for each patient. The preventive services including dental cleaning, fluoride treatment and sealants are provided at the first visit. Future appointments may involve restorative care (fillings) if needed. All patients are given recare appointments based on their caries risk assessment - likelihood of getting tooth decay.

The mission of Commonwealth Mobile Oral Health Services (CMOHS) is to provide comprehensive oral health care to underserved children, adults and elders of Massachusetts. CMOHS strategically plans and provides education and oral health services to individuals who are victims of oral health disparities, are faced with barriers accessing dental care and who do not have a dental home.
